We believe in providing exceptional care in a quality setting to optimize your healing journey.  In order to provide the best care possible, we will need a referral or medical records from your provider.  We may also need an authorization from your insurance company.  Below is an example of insurance companies that require a referral, authorization, or both.

  • Referral and Authorization is required for procedures. A consult may be needed prior to your procedure.
  • Referral and Authorization is required for all HMOs.
  • Referral and Authorization is required for all AHCCCS/Medicaid plans.
  • Referral and Authorization is required for Tricare and TriWest plans.

Your appointment with Center for Neurology and Spine will be easier if you come prepared with the appropriate forms completed. Please also remember to bring any imaging reports, recent laboratory studies, or any other diagnostic testing with you. We appreciate your help in this matter.

Please contact us at 602-482-2116 between 8:00AM and 4:30PM Monday-Friday to obtain information on our clinicians and services, schedule and confirm patient appointments, or receive assistance in the resolution of any service-related issues.

When contacting us to schedule your patients, please have the following information readily available:

  • Patient demographic information
  • Reason for visit
  • Any special needs that will require facilitation once the patient arrives
  • Patient insurance information

We will work with your patient to complete registration process and to schedule an appointment at the patient’s convenience. You will receive a dictation/note for each visit or procedure that is performed on your patient. Together as a team, we can achieve the best possible outcome for our mutual patients.

Please note, CNS and/or its providers may not be contracted with all plans within each carrier. Please check with your insurance to verify.
